Benefits (including Tax Credits, Housing/Council Tax, JSA, ESA, DLA)

Please bring with you all relevant documents as well as your National Insurance Number, date of birth and full home address.

HMRC / Tax

Please bring with you all relevant documents as well as your National Insurance Number, date of birth and full home address. 

Telford and Wrekin Council (including Housing, Highways, Street Lighting, Refuse Collections, Business Rates)

Your full home address, any applicable reference numbers and an outline of your concerns will help to ensure a quicker response.

NHS

If your query relates to the NHS, please provide your full name, address, date of birth, NHS number, GP name & surgery address and any additional information you feel will help Lucy to enquire about your issue.

CSA

Please provide your full name, address, date of birth, National Insurance Number, your former partner's name & date of birth and the names and dates of birth for any children involved.

Immigration

Your full home address, your Home Office reference number, your date of birth, country of origin and current status within the UK. We may also need to know when you arrived in the UK and how you arrived.

Visas

If the person applying for the Visa is currently out of the country, please provide as much written information as possible, including the applicant's full name, date of birth, address, reason for applying for a Visa, how long they intend to be in the UK and any family links the applicant has to the UK.
